FORMS:

Troop medical form: The troop needs one of these on file each year from each scout. In particular, one must be on file for each scout going on a campout. Only the class one section needs to be completed. A more detailed form is required for summer camp. Completed forms should be given to the scoutmaster or any of the assistant scoutmasters.

Summer camp medical form: A different form is required for Summer Camp. It ia a four-page medical form. These will be collected in advance of camp. Adults who plan to attend must also fill out this form.
