FORMS:

Troop medical form: The troop needs one of these on file each year from each scout. In particular, one must be on file for each scout going on a campout. Only sections A and C need to be completed for teh troop. A more detailed form is required for summer camp. Completed forms should be given to the scoutmaster or any of the assistant scoutmasters. Summer camp medical form: The full 3 page version of the medical form is required for Summer Camp. These will be collected in advance of camp. Adults who plan to attend must also fill out this same form.
